Equifax Existing Member Account Set Up Process

If your firm (or other departments or divisions of your firm) is already an Equifax member then your firm needs to simply follow the steps below. There are no additional Equifax membership fees if your firm is already an Equifax member.

  1. Complete an Equifax Membership Addendum: Existing members of Equifax should contact the Equifax Customer Service at 1(800) 796-6573 to request an Equifax Membership Addendum.
     
    • As an existing Equifax member, you will not be required to pay an additional membership fee, nor is an additional site visit required.
    • You will need to complete a Membership Addendum to receive the PERSONA PLUSTM Report service through Web CRD.

      Note: Equifax will notify NASD of your firm's completed Membership Addendum to receive the PERSONA PLUS Report service through Web CRD.
  2. Update your firm's Web CRD Account Administrator(s)' privileges: You must be entitled with the "Equifax Employment Screening Report" privilege on Web CRD before you can access the Equifax application.
     
    • Complete the NASD Entitlement Modification Form for NASD Member Firms (Web CRD). Select the addition of the "Equifax Employment Screening Report" privilege. Please complete a separate form for each Web CRD Account Administrator for whom you are requesting this privilege. Send the completed form(s) to NASD via fax or mail (fax number and address provided on the form).
    • NASD will contact your Web CRD Account Administrator(s) once the privilege has been set in Web CRD.
    • Web CRD Account Administrator(s) with the Equifax Employment Screening Report privilege should entitle designated users at their firm with this privilege. (Please note: Users must be set up in both Web CRD and the Equifax ePORT® application in order to access the PERSONA PLUS Report service.)
    • Equifax will provide your firm with a new customer member number and security digits once NASD Entitlement has set the Equifax privilege on your Web CRD Account Administrator's account. You will need your new Equifax customer member number and security digits to complete the next step, Equifax's online registration process.
  3. Complete the one-time online registration process with Equifax: The online registration process is only done once for your firm.
     
    • The individual who completes this online registration process will automatically be set up as one of your firm's Equifax Administrators. (This person may also be one of your firm's Web CRD Account Administrators, which is recommended but is not required.)
    • To complete this online registration, access the Equifax application through Web CRD by selecting the "Equifax Employment Screening Report" link from the Web CRD site map.
    • When you begin the online registration process with Equifax, you will be asked to enter the new customer member number and security digits Equifax provided to you.
    • Complete the online registration process. Please refer to the Equifax Administrator Guide provided by Equifax for detailed information on how to complete the online registration process and for information on how to perform account administration activities such as creating user accounts.
    • Once the online registration process is complete, you may create accounts for other Equifax Administrators (preferably two) for your firm, and set up Equifax users at your firm.

      Important Points:
    • As a security feature, Equifax Administrators are not able to request PERSONA PLUS Reports. If you are an Equifax Administrator, you must create a user account for yourself in order to be able to request PERSONA PLUS Reports.
    • There is approximately a 24 hour delay following the online registration process before your firm may request PERSONA PLUS Reports.
